[226] The petition of Alfred Leurquin hereby filed, respectfully showeth, First, My place of residence is 1808 Monroe Street, Green Bay, Wis.

[228] Third, I was born on the 12th day of November, anno Domini 1858, at Temploux, Belgium.
[229] Fourth, I emigrated to the United States from Antwerp, on or about the month of October anno Domini 1870, and arrived in the United States, at the port of Detroit, on the month of October anno Domini 1890 on the vessel "Grand France".
[230] Fifth, I am married. My wife's name is Virginia; she was born on the 15th day of June, anno Domini 1860, at in Brown County, Wis, and now resides at Green Bay, WI. I have 7 children, and the name, date and place of birth, and place of residence of said children is as follows: Exavier born Nov 6 - 1882 Alvinia born Jan 24 - 1885 Jennie born Dec 30 - 1887 Mary born Apr 26 - 1890 Alphonse born Jul 28 - 1893 Victoria born Jul 5 - 1898 William born Jun 13 - 1901 All born in and reside in Green Bay, Wis.
[231] Seventh, I am not a disbeliever in or opposed to organized government or a member of or affiliated with any organization or body of persons teaching disbelief in or opposed to organized government. I am not a polygamist nor a believer in the practice of polygamy. I am attached to the principles of the Constitution of the United States and renounce absolutely and forever all allegiance and fidelity to any foreign prince, potentate, or sovereignity, and particularily to Albert I, King of Belgium of whom at this time I am a subject, and it is my intention to reside permenantly in the United States.
