Dr. Elijah Randolph Smallwood was born on 11 April 1819 in Claiborne County, Tennessee. About 1860 he was a doctor. Bessie claims that Elijah was educated to become a doctor. According to his disability affidavit of 1891, Elijah states that he was a "farmer and practitioner of medicine" before his enlistment into the services of the United States (the Civil War). He also states that since his discharge, his occupation had been that of a farmer and MD, In these same papers, when asked what kind of treatment he had received for his disease, he states, "Have not been treated by any physicians since my discharge. Being a physician myself, I have continually used remedies of my own prescription." It is not known what kind of medical training Elijah received, when or where he was trained. Bessie recalls a time in her young life when diptheria was prevalent. "Granddaddy Smallwood" was the only doctor in Dorton, Kentucky for twenty miles. She feared his presence when he came to doctor the family, and would hide under the porch. He served in the military during the Civil War in Company D, 39th Kentucky Volunteers from 6 September 1862 to 15 February 1864. About a year and a half into the Civil War, Elijah volunteered to serve as a soldier in the Army of the United States. He was a private in Company D of the 39th Regiment of Kentucky Volunteers under the command of Captain Martin Thornburg. He was 43 years of age at the time. On Feb 15 1864, Elijah was granted a disability discharge from the army due to bronchitis and partial paralysis of the right side. This was a result of his becoming overheated in battle in Nov or Dec of 1862 near the mouth of John's Creek in Johnson County, Kentucky, followed by exposure. ". . . . in order to make escape from being captured, had to wade the river and lay out." (quoted from disability affidavit). Elijah was described as "six foot two inches high, black eyes, black hair, and dark complexion." in September 1862. In 1869 he was a school teacher for about three months in Indian Creek, Pike County, Kentucky. He was an a farmer in Dorton, Pike County, Kentucky. Elijah retired after June 1890. Under the Act of Jun 1890, Elijah applied for and received a pension of $12 per month from the U. S. Pension Agency. By the time of his death, this had been raised to $30. On the affidavit he states, "I have suffered off and on all along with paralysis of the left shoulder and arm, and has also partially got into my right arm causing me to act in a palsied manner. Also bronchitis and catarrh of heart (for which I was treated by Dr.) myself." He died on 11 June 1898 at the age of 79 in Dorton, Pike County, Kentucky. There is not much definitive information about his early years. According to Bessie, he was raised by Washington Ratliff's family as a sister to his future wife, Mary Elizabeth. However, Alex Osborne claims that he was raised by his maternal grandparents, David and Jane Osborne Edwards. His mother, Edith, also lived with them. Alex writes, "When E. r. Smallwood grew up, he spent much time in John's Creek, Kentucky. While there he married Washington Ratliff's daughter. He went to Dorton Creek and laid a patent on all the land known as the E. R.
