Hazel Martha (Anderson) Fields about 1926. Hazel was born on 17 Sept 1894 in Guthrie, Logan County, Oklahoma. She married Floyd Fields on 5 Aug 1915 in Parkland, Lincoln County, Oklahoma. They had two children, Alva and Lester. Floyd and Hazel made their home in Arizona, and Hazel, who died on 30 Aug 1976 in Phoenix, is buried in Phoenix, Maricopa County, Arizona.
