DECLARATION FOR INVALID ARMY PENSION.
----------------------------------
State of Indiana, County of Morgan, ss:
On this 9th day of December, One Thousand Eight Hundred and Seventy-five, personally appeared before me, Willis Record Clerk of the Circuit Court within and for the County and State aforesaid, Balaam Bolin, aged 47 years, and a resident of Martinsville, County of Morgan, in the State of Indiana, who being duly sworn according to law, declares that he is the identical Balaam Bolin who enlisted in the service of the United States at [blank] County of Monroe and State of Indiana, on the 12 day of August, 1862, as a Private in Company "B", commanded by Captain Denny, in the 67" Regiment of Ind. Vols., in the War of 1861, and honorably discharged at Indianapolis, State of Indiana, on the 6" day of May, 1863, that while in the service aforesaid, and in the line of duty as a soldier, at a place called [blank] in the State of Kentucky, on the [blank] day of Sept'r, 1862, From exposure he contracted chronic diarrhoea and disease of the heart which continue and greatly incapacitate him for manual labor.
Received hospital treatment as follows: Camp "Morton" near Indianapolis.
He has not been in the military or naval, marine or civil service of the United States since the 6 day of May, 1863. Since leaving the service he resided mostly at or near Morgan Co., Ind. His occupation has been Farming. When enrolled he was a Farmer. And for the purpose of prosecuting his claim he hereby appoints P. H. FITZGERALD, of Indianapolis, Indiana, his attorney in fact, with full power of substitution, and with authority to receive his Pension Certificate.

Also personally appear James E. Asher and James B. Farr, of the County of Morgan and State of Indiana, whom I certify to be respectable and entitled to credit, and who being by me duly sworn, say that they were present and saw Balaam Bolin subscribe to the foregoing declaration. And they further say that from the appearance of the applicant, and their acquaintance with him, that he is the person he represents himself to be, and that they have known him for Twenty (20) years last past, and during that time his habits have been good, and his occupation has been that of a Farmer.
They further state that they have no interest in the prosecution of this claim.

Sworn to, acknowledged and subscribed before me, this 9th day of December 1875, and I hereby certify that the contents of the foregoing declarations of claimant and affidavit of witnesses was made known to each of them before administering the oath; and that I have no interest, direct or indirect, in the prosecution of this claim.
