GENERAL AFFIDAVIT

Note:-The affiants should state how they gain a knowledge of the facts to which they testify.

 

STATE OF Iowa

COUNTY OF Cedar}ss.

In the matter of David S. Hepner

ON THIS 10 day of March A. D. 1880, personally appeared before me a Notary Public in and for the aforesaid County, duly authorized to administer oaths[,] David Clark[,] aged 32 years, a resident of Tipton in the County of Cedar, and State of Iowa, whose Post Office address is Tipton Cedar Co. Iowa ["and ... address is" information for a second affiant lined out] well known to me to be reputable and entitled to credit, and who being duly sworn, declare in relation to aforesaid case, as follows[:]

I am personally acquainted with David S. Hepner, have known him for several years, have worked with him during the last three years, and I know that he is afflicted with the chronic Diarrhea and has been so afflicted for the past 3 years[.] that he is at least one half disabled from obtaining subsistence by manual labor. My knowledge is derived from my acquaintance with him & from having worked in same field with him at different times during the past 3 years.

I further declare that I have no interest in said case, and I am not concerned in its prosecution.

Sworn to and subscribed before me this day by the above-named affiant and I certify that I read the said affidavit to said affiant, and acquainted him with its contents before he executed the same[.] I further certify that I am in nowise interested in said case, nor am I concerned in its prosecution; and that said affiant is personally known to me, and that he is a credible person.
