[Handwritten]

State of Indiana

Morgan County } ss

Personally appeared before me, Willis Record, Clerk of the Circuit Court of the County and State aforesaid, Harvey Baker, and Jacob C. Lafever, and Joseph Lewis residents of Morgan & Monroe Counties, the post office address of the said Baker and the said Lewis being Martinsville, Indiana, and the post office address of the said Lafever being Paragon, Indiana, all of whom I certify to be respectable persons entitled to full credit and belief, who upon their oaths state that they were well acquainted with one Henry A. Bastin deceased, who was a Private of Company "H" of the 33rd Regt of Indiana Volunteers in the war of 1861, and who died on the 26th day of August 1873. That they were intimate friends and acquaintances of the said Bastin for fifteen years prior to his said death. That prior to the ["said" lined out] enlistment and at the time thereof, of the said Bastin into the service of the United States, he, the said Bastin, was a strong, robust, sound, and able bodied man free from any pulmonary desease or other disease whatever. That when he returned home at the time of his discharge from the said service he, the said Bastin was broken down in health and appeared to be afflicted with rheumatism and lung disease and so continued until his death was produced from the effects of the said disease at the date aforesaid. After the return of the said Bastin from the said service, he was drafted into the service of the United States in February 1865, and was returned into the said Company and regiment, but as these affiants are informed, the said Bastin was almost wholly unfit for duty during ["his" lined out] the time of his second service. These affiants, unless as heretofore stated, testify from their own personal knowledge. They further say that they have no interest whatever in the claim for pension of Elizabeth Bastin, which this affidavit is sought to establish.
