CERTIFICATE OF DISABILITY FOR DISCHARGE

(To be used, in duplicate, in all cases of discharge on account of disability.)

Henry A Bastin Private of Captain Jas. E. Burtons Company, (H) of the thirty third Regiment of ["United States" lined out] Ind Vol's was enlisted by Jas E. Burton of the 33rd Regiment of Ind Vols at Paragon[,] Morgan County[,] Ind on the 23rd day of August 1861, to serve 3 years; he was born in Morgan County in the State of Indiana is Thirty years of age, 6 feet " inches high, Light complexion, Blue eyes, Light hair, and by occupation when enlisted a Farmer[.] During the last two months said soldier has been unfit for duty 60 days.

I CERTIFY, that I have carefully examined the said Henry A Bastin Priv of Captain Jas. E. Burtons Company, and find him incapable of performing the duties of a soldier because of (Here consult par. 1134, p. 245, and directions on Form 12, p. 269, Med. Dept. Gen. Reg.) [?] in the left leg and shoulder, occasioned by a severe and long continued attack of Rheumatism, which not only disqualifies him from performing the duties of a soldier, but renders him unable to gain a subsistence by manual labor.. Degree of "Disability" three fourths
