GENERAL AFFIDAVIT.

State of Indiana, County of Morgan, ss.:
In the matter of the Pension Claim of Elizabeth Bastin No. 213,979. widow of Henry A. Bastin Priv. Co (H) 33rd Ind Vols
Personally appeared before me, a Justice of the Peace, in and for said county, and State William Collier, aged 59 years, P. O. address, Paragon, County of Morgan, State of Indiana, a person of lawful age, who, being duly sworn, declares in relation to the aforesaid case as follows:
Was well acquainted with Henry A. Bastin. all time after he was discharged from the Service untill his death lived a near neighbor to him all the time[.] I was with him during his last Sickness almost evry day. He Suffered from Shortness of breath and Smothering Spells[.] Said Baston could not exert himself in any manner without Suffering from Smothering Spells and He was afflicted with Rheumatism of left hip. He was not able to make a hand at work at no time after his discharge and the last year of his life he was not able to do any work to do himself justice
I further declare that I have no interest in said case [and] am not concerned in its prosecution.
