GENERAL AFFIDAVIT
State of Indiana, County of Starke, ss:
In the matter of Pension Invalid Claim Certificate No. 362.802. of Matthias T. Hepner, Co. D, 19th Reg't Ind. Volunteers.
Personally came before me, a Clerk of Cir. Court in and for the aforesaid County and State, Joseph Phillips, aged 71 years, a citizen of Knox, No. [blank] Street, County of Starke[,] State of Indiana, well known to me to be reputable and entitled to credit, and who, being duly sworn, declares in relation to the aforesaid case, as follows:
I was a member of ["same company and Regiment as" lined out] Company D 29th Indiana Regt. of Indiana vol. Infantry. The above Claynant Mathias T. Hepner was a member of the same Company and Regiment afforesaid as fur as I no he was in good health and free from diseas at date of enlistment. On the day of November 1861 he was excused from duty on account of Camp Diarhea and kidney complaint at Camp Nevin vilage of Nolan Kentucky[.] I was Seargent of the Company and no he was releaved from duty by the Docters sertificate in May 1862[.] Said Aplicant was Sent North to Louisville Kentucky from [?]ichberg landing Tennesee with Tiphord neumonia and discharged at Lewsville Kentucky on 27 of June 1862 on acount of disability as reported to said Regiment[.]
This is my written statement in making the Same I was not aided or Prompet nor did I use any written or Printed formula Prepared or written by any other Person not atached as an Exhibit to this testimony

Sworn to and subscribed before me this day of the above-named affiant, and I certify that I read said affidavit to said affiant, including the words [blank] erased and the words [blank] added, and acquainted [blank] with its contents before he executed the same. I further certify that I am in nowise interested in said case, nor am I concerned in its prosecution; and that said affiant is personally Known to me and that he is a credible person.
