It included a Family Group Sheet for the Carl Unoff Schold Johnson family. "I'm enclosing a form for your immediate family and one that I have gathered together for the Johnson line. Please fill it out and return it for your family and keep the other one, but if you can give me any information for the blank spaces on the Johnson one I would love to have the information and its sources."
